Grace Athletics Sports Camp is Tuesday, July 14 - Friday, July 17 from 9:00am - 12:30pm. The camp is designed to introduce kids to multiple sports, learn about teamwork and sportsmanship and develop a love for playing the games! Registration is open for boys and girls who have completed Kindergarten through completed 5th Grade. Cost is $95 per participant. Deadline to register is Monday, June 15.

By completing this registration form, I, the parent or guardian, do hereby grant permission for my child to participate in sports and related activities with Grace Evangelical Church (GEC). I understand that due to the physical contact associated with this activity, there is a possibility that my child may sustain physical injury while participating. In order that my son/daughter may receive any necessary medical treatment he/she may need, I hereby authorize the coach or other supervising adults to obtain it, and hereby hold GEC and its representatives harmless in the exercise of such authority. I further release GEC and its representatives from any claims for personal illness or injury that my child may sustain. By entering this space, you are consenting to photography and video of your exact likeness to be used by the church for streaming and promotional purposes. Anyone under the age of 18 is consenting to the ambiguous use of their likeness for streaming and promotional purposes of the church. Use of anyone under the age of 18 for streaming and promotional purposes with exact likeness must get explicit permission from a parent or legal guardian given to a staff member of the church. If you object to any use of your exact likeness to be used by the church, please let a staff member know.
